The table provides information on how to contact organizations to obtain forms. Expect to spend between $0.21 and $1.00 per form, depending on the quantity and the type of form you order, e.g., snap-outs or continuous.

What is a Manifest Tracking Number (MTN)?
A manifest tracking number (MTN) is an alphanumeric identification number (i.e., a unique three-letter suffix preceded by nine numerical digits), which is pre-printed in Item 4 of the manifest by an approved registrant.
I am interested in applying to EPA to print the manifest. How do I select a suffix?
In your application under 40 CFR 262.21(b), you must propose a unique three-letter suffix that will be used to pre-print a unique MTN on each manifest. You should consult the Table of Approved Registrants to identify MTN suffixes that have been approved already. You can propose any three-letter suffix that has not been approved.
