Dear Sir or Madam:

This request covers the time period from January 1, 1996 until the date of this letter.

The term "document" as used herein, is used in the broadest sense, and means, without limitation, any written, printed, typed, photostatted, photocopied, microfilmed, handwritten, recorded, or otherwise reproduced communication or representation, whether composed of letters, words, numbers, pictures, sounds, symbols, or any combination thereof, including but not limited to documents, letters, memoranda, electronic mail, phone records, and handwritten notes.

If you cannot send a copy of any of the documents requested, I request a statement in writing of the reason that the document is being withheld.

Thank you for your prompt attention to this matter. After you have identified the individual who will be performing the search, please call and tell me his or her name in case I need to modify my request.
